Insulin-related hypoglycemia: Did we cause this problem?

Approximately 100,000 emergency department (ED) visits and 30,000 hospitalizations occur annually as a result of insulin-related hypoglycemia and errors, with the greatest risk occurring in patients aged 80 years or older.
